Please complete this form if your player missed Picture Day on January 13th, but is available on Saturday, January 20th for an individual photo, in order to be edited (photoshopped) into the final team photo captured last week.

Forms must be in by Friday, January 19th. Shoots will be scheduled 15 minutes before your player’s designated start time. Further instructions will be communicated after completion.
